Case Management

Case Management services are designed to ensure access to a range of services for the client and identified family members. Services involve the completion of a comprehensive needs assessment to identify both short and long-term services needs. Upon completed of the assessment, staff facilitate the completion of a corresponding service plan. These services enhance a client's ability to secure transitional or permanent housing based on their individual level of readiness. Ongoing referrals, monitoring and follow-up help the client achieve his/her goals.
